A threesome is the most common sexual fantasy of all, and gay men are the ones most likely to turn it into reality. Still, a first threesome rarely goes the way it played out in your head. The threesome guide by TomRockets fixes that. You get five rules that rescue any three-way date, and you learn what research actually knows about group sex, open agreements, and jealousy. We draw on studies with more than 4,000 respondents. We use a cohort of 392 men and their 1,033 documented group sex experiences. And we add [couples](https://gay-blog.tomrockets.com/en/bdsm-for-couples/ "couples") research with 161 gay couples. Sounds like a seminar, but it is the shortest route to a threesome everyone enjoys.

## Basic facts about threesomes, planning is half the sex date

A threesome is, plainly defined, a sexual interaction between three people in which at least one person engages physically with both of the others. First the good news: your wish puts you in excellent company. Sex researcher Justin Lehmiller surveyed 4,175 Americans for his book „Tell Me What You Want.“ 95% of men and 87% of women said they had fantasized about sex with multiple partners. The part that matters for you: according to Lehmiller, non-heterosexual men had acted on their group sex fantasies most often, at 25%. In Lehmiller’s data, non-heterosexual men also reported the most positive experiences.

Large population studies confirm the threesome as perfectly normal. Herbenick and colleagues (2017) surveyed a nationally representative sample in the United States. The result: 17.8% of men and 10.3% of women had already had a threesome, and 11.5% of men and 6.3% of women had engaged in group sex. And Joyal and colleagues (2015) had 1,516 adults rate 55 sexual fantasies. Only 2 of the 55 fantasies were statistically rare, and only 9 more were unusual. The large majority of fantasies is common, much of it very common. Fantasizing about a threesome is about as exotic as coffee for breakfast.

Many threesomes fail at the same point despite this ubiquity. Lehmiller supplied the answer along with the data: most people have no script for how a threesome should unfold. The following five rules deliver exactly that missing script.

### 1. State expectations before meeting

No matter the constellation, three strangers from Grindr, a couple plus a guest, or some other setup: the best threesome starts before the first kiss with an honest statement. All three state what they are into and what they do not want. Condoms or PrEP, [poppers](https://gay-blog.tomrockets.com/en/poppers-effects/) yes or no, sleepover or not: all of that belongs on the table in advance. The rules talk sounds bureaucratic, but it is arousal insurance. When expectations are clear in advance, no surprise no pops up mid-session.

Expectation agreements of this kind are standard among gay men, by the way, not the exception. Hoff and colleagues (2010) surveyed 566 gay couples in the San Francisco Bay Area. The majority had explicitly discussed their relationship agreements, and only 8% of couples held conflicting ideas about what had actually been agreed. Neilands and colleagues (2010) sum up the finding in one sentence: sexual agreements are ubiquitous among gay men. And they work: couples more invested in their agreement were more likely to keep it and less likely to have unprotected sex outside the relationship.

Yet the honesty gap shows up in the PrEP era. MacGibbon and colleagues (2020) analyzed data from 21,593 men: nearly 40% of PrEP users in relationships had an agreement allowing condomless sex with others. Another 34% had such sex without any agreement at all. Anyone already on PrEP who has never discussed outside contacts is in good company. Men without an explicit agreement give up exactly the benefit an honest agreement provides.

### 2. The most important thing, safety in threesomes

Three bodies in one bed mean more possibilities, but also more combinations you need to think through in advance. Safer sex means protecting yourself and your partners from [sexually transmitted infections](https://gay-blog.tomrockets.com/en/health-en/sexually-transmitted-infections/ "sexually transmitted infections"), for example through condoms, through HIV pre-exposure prophylaxis (PrEP), or through regular testing. The core rule stays simple: talk about safer sex before the pants come off. PrEP, condoms, recent test results. Feel free to ask a stranger about his last test. Anyone who finds that question uncool has disqualified himself as a threesome candidate anyway.

Knox and colleagues (2020) measured the actual risk of group sex more precisely than almost any other study. In the Amsterdam cohort of men who have sex with men (MSM), 392 men reported a total of 1,033 group sex experiences between 2015 and 2018. The researchers found four types: familiar (29%), intimate (27%), impromptu (36%), and party (8%). The intimate type, typically a threesome in a private place without drugs, showed markedly lower rates of risky practices. The party type, with larger groups, mostly unknown partners, and advance planning, showed elevated rates, including of condomless anal sex. The study’s own conclusion is sober: some types are riskier for sexually transmitted infections, others for the immunodeficiency virus (HIV).

The message of the Knox study is not „group sex is dangerous.“ It is this: the setting decides. A planned threesome with clarified expectations matches the study’s intimate type. The party type with larger groups and rotating partners sits at the other end of the spectrum, especially when [chemsex](https://gay-blog.tomrockets.com/en/chemsex-2/) is involved. You steer the risk through the setting, not through abstinence. And if you ever want to go bigger, our guide to [planning a gay orgy](https://gay-blog.tomrockets.com/en/gay-orgy-plan/) has the matching rules.

### 3. Top or bottom & to kiss or not to kiss

Three men in bed means three roles in your head. Who is a top, who is a bottom, and who wants both belongs on the table. There is hardly a bigger mood killer than discovering that all three of you would rather receive. So clarify the role question beforehand, ideally while chatting or meeting. And if you are actually versatile, say so instead of being surprised later.

Couples face a second layer. First agree between yourselves what works for both of you, then communicate the result explicitly to the third. Kissing is the classic here: some couples allow everything in a threesome except kissing the guest. Kissing bans of this kind may look arbitrary from the outside, but for many couples they are the emotional boundary that counts. They are legitimate as long as all three know them before things get started.

What many underestimate: clear rules do not harm the mood, they make it possible. Couples research has pointed the same way for years. Couples with explicit agreements are not less happy than others, quite the opposite. In their review, Conley and colleagues (2017) found few differences in relationship functioning between monogamous and consensually non-monogamous couples.

### 4. Pay attention to the chemistry between the parties

Threesome math is unfair by design: if just one of the three does not get into gear, the whole thing tips over. Sex with three people creates three pairings, which means more points of friction but also more possibilities. That is why chemistry is not a side issue but the main ingredient. If the vibes are off for one of you, do not try to force them. Chemistry matters most in the moment itself: if one of the three cools down, slow down, ask, switch the constellation.

Deliberately shifting attention is a proven tool. Give one of them your full attention first, then the other. Feel free to say out loud what you see and what turns you on about it. If you lack the words, our [dirty talk guide](https://gay-blog.tomrockets.com/en/dirty-talk-2/) has concrete sentences for exactly these moments. If you notice the chemistry crackling between the other two right now, use it. Switch into spectator mode for a round, watch, cheer them on. Watching is no consolation prize in a threesome; for many men it is a kick of its own.

Also mind your own expectations: not every threesome will be a bullseye. Lehmiller describes the threesome as the fantasy most likely to go wrong when acted out. Reality differing from fantasy is no argument against a threesome, but an argument for relaxed expectations. A threesome is allowed to be simply fun, chaotic, or educational.

### 5. Giving feedback

The threesome does not end with the last orgasm but with the first conversation afterwards. For couples especially, the debrief decides the future of the experiment. Talk about what was hot and what felt strange. Also settle whether you want a repeat, and if so, under which rules.

Jealousy belongs in this conversation, not under the bed. And here research has a finding that surprises many: Parsons and colleagues (2012) surveyed both partners of 161 gay couples, 322 men in total. 52.8% of the couples lived monogamously, 13.0% open, 14.9% monogam-ish, and 19.3% without a shared agreement. The result: the type of agreement was not associated with sexual satisfaction, communication, or frequency. But monogamous men reported significantly higher sexual jealousy. Openness did not make men in this sample more jealous; on average it made them calmer.

A German sample recently refined this picture: Morar-Bolba and colleagues (2025) asked 1,623 people about relationship style and satisfaction. Monogamous participants did report jealousy less often. Yet they experienced it as more distressing than any of the consensually non-monogamous groups. The decisive drivers of satisfaction were need fulfillment and good communication, not the relationship model. Giving feedback is no ritual for the faint-hearted; it is exactly the mechanism the satisfaction research measures. Darbes and colleagues (2014) followed 566 couples through 6 measurement waves over 3 years. Among seroconcordant couples, those investing more in their agreement and their communication had measurably less unprotected anal sex outside the relationship.

## Where to look for a threesome

Finding the third is the logistical core question of any threesome: venue and pre-selection decide the outcome. You can search in the real world or online. Both routes work, and both have their own strengths. The short answer: apps like Grindr deliver the largest selection and the best pre-filtering by profile. Bars, clubs, and saunas deliver the instant face-to-face chemistry check instead. The final decision always happens when you actually meet.

### Three-way candidates in bars, clubs and discos for gays

Much of the gay community looks for sex partners online these days. Yet bars, clubs, and saunas have one decisive advantage: you see immediately who is in front of you, no filters, no Photoshop. As a couple, you can get to know a candidate over a drink and test directly whether the chemistry works. The face-to-face chemistry check is worth more in a threesome than any profile picture.

The downside: in a bar you discuss expectations more or less on the fly. If you pick someone up there, do not skip the rules talk anyway, even if it happens in a quiet corner instead of at the breakfast table.

### On the internet for a threesome

Apps like Grindr, Romeo, or Hornet are the fastest way to a third. As a couple, state in the profile that you are looking and what a candidate should bring. Clear messages filter the candidates and speed up the search. Separate profiles work too, but then the search should appear on both.

An honest profile is no contradiction to selling yourselves. Both partners in the pictures, a clear message, no surprises at the meeting. No matter how explicit the chat was: everyone’s [consent](https://gay-blog.tomrockets.com/en/bdsm-consent/ "consent") is confirmed live at the meeting. What sounded settled online needs one more yes in real life. Then nothing stands between you and a hot three-way.

## FAQ

### How common is the fantasy of a threesome?

Very common. In Lehmiller’s survey of 4,175 Americans, 95% of men and 87% of women had fantasized about sex with multiple partners. In the population study by Herbenick and colleagues (2017), 17.8% of men and 10.3% of women had already experienced a threesome.

### Is a threesome in a relationship a good idea?

Research says: it depends on the agreement, not the model. Parsons and colleagues (2012) found no differences in satisfaction and communication between monogamous and open couples among 161 gay couples. The surprise: monogamous men reported the highest sexual jealousy. Openness did not make the couples in this sample more jealous.

### How do I find a third for a threesome?

Fastest via apps like Grindr or Romeo, most directly in bars, clubs, or saunas. Online the rule is: honest profile, clear message, consent confirmed again at the meeting. Offline the chemistry check over a drink beats any profile picture. Both routes lead to the goal, as long as expectations are clarified before the date.

### Do I need condoms in a threesome if I take PrEP?

PrEP protects against the immunodeficiency virus (HIV), but not against other infections like syphilis or gonorrhea. The Knox study (2020) also shows that risk depends heavily on the setting. The intimate type, a planned threesome in a private place without drugs, had the lowest rates of risky practices. The party type with unknown partners had the highest. Clarify protection and status beforehand, whichever combination you choose.

### What do I do if jealousy comes up during a threesome?

Pause briefly, make eye contact, take a break if needed. Afterwards the topic belongs in the debrief, not under the bed. The data from Morar-Bolba and colleagues (2025) show: it is not the relationship model but communication and need fulfillment that decide satisfaction.

### Who sets the rules in a threesome?

All three, ideally beforehand. As a couple you first agree between yourselves what is on the table and what is off it, and you share that explicitly with the third. The third is not an accessory but a playmate with his own [boundaries](https://gay-blog.tomrockets.com/en/bdsm-limit-boundaries/ "boundaries"), which count exactly as much as yours.

## Conclusion: the best threesome is the one with a script

A threesome among gay men is normality itself. Fantasies of sex with multiple partners are the most common in the world, and men who sleep with men act on them most often. According to couples research, a threesome is no risk to the relationship as long as the agreements hold. Plan your threesome like a good date: clarify expectations, discuss safety, assign roles, listen to the chemistry, and talk afterwards. Then the world’s most common fantasy ends as a genuinely good memory.
