Opening the small hole to fist size? An introduction to **[fisting](https://gay-blog.tomrockets.com/en/fisting-en/ "fisting") stretching** and the art of active relaxation. It pays to start with a look at the two muscles everything here revolves around. The external sphincter consists of striated muscle, the kind you control voluntarily, like an arm. The internal sphincter consists of smooth muscle and does not obey your will. And it is the more important of the two: measurements by Frenckner and Euler (1975) show that it carries about 85 % of the resting pressure in the anal canal. Your seal is, at its core, a muscle you cannot command directly. The good news: there is a reflex that opens it for you, and you can learn to trigger it.

## Open Sesame!

The rectoanal inhibitory reflex is a protective mechanism of your body: when the rectum fills, the internal sphincter goes slack by itself. When your rectum fills, stretch receptors in the bowel wall register the pressure. Then something decisive happens: the internal sphincter relaxes reflexively. Physicians call this the rectoanal inhibitory reflex. In everyday life it allows the bowel to empty at all. And it works more subtly than you might think. As the muscle relaxes, the pressure in the anal canal drops far enough for rectal content to touch the sensitive zone. That way the body takes a sample of whether gas or solid is on its way (described by Duthie and Bennett, 1963).

Frenckner and Euler measured how strongly this reflex works: with sudden, substantial filling of the rectum, the internal sphincter’s share of the pressure drops from 85 % to about 40 %. The muscle gives way without you doing anything.

This is exactly what the oldest trick of stretching training builds on: press lightly during the exercise, as if having a bowel movement. You simulate fullness and borrow the reflex that opens your internal sphincter. A full bowel is not much fun for most people during fisting, but the reflex can also be triggered when you are douched and empty.

## Fisting stretching ability

So how stretchable is the sphincter? The honest answer: nobody has ever measured it precisely, at least not in the context this is about. What research offers are two clinical observations that combine into a usable picture.

The first: slow, controlled stretching seems to sit well with the muscle. In studies on fissure treatment, where patients stretched over weeks with dilators, no cases of incontinence occurred (Schiano di Visconte, 2006). Self-controlled stretching with one’s own fingers showed high healing rates there (Gaj, 2017). The important footnote: these studies did not re-examine the muscle by ultrasound. Small damage without symptoms cannot be ruled out.

The second observation shows the flip side: fast, forced stretching verifiably destroys the muscle. You will find the figures in the section on overstretching. Your working title for training may therefore read: the sphincter can be stretched far, as long as you do it slowly and carefully.

## Training on the organic level

The basic principle of stretching training has three steps: stretch to the load limit, linger there, then go one millimeter further. There are several ways to stretch the involuntarily controlled sphincter. Let us start with training on the organic level. The principle is very simple. As with building muscle in the gym, you always go to the load limit and stay there. In the gym that means: strain until you cannot go on. In preparing for fisting: stretch until it pulls a little, but does not hurt badly yet. Linger at this limit and breathe deeply. Hold it until you notice the unpleasant stretching sensation fading. Then, and really only then, stretch the sphincter one millimeter further. Linger again, wait for the feeling to fade, and stretch another millimeter, or just a fraction of one.

## With this you can ease the stretch

Seven techniques help you ease the stretch, from warmth to breathing to focusing on the sensation itself.

- By practicing in the bathtub, after a [shower](https://gay-blog.tomrockets.com/en/sex-in-the-shower/ "shower"), or in the sauna. It is warm there, and the body is relaxed.
- By pressing lightly as if having a bowel movement. This triggers the reflex that opens the hole from the inside.
- By breathing deeply: in pain we tend to hold our breath, an automatic reaction of the nervous system. You counter it by consciously breathing in and out, slowly and deeply. That promotes relaxation.
- By concentrating on the pain: as soon as pain sets in, you automatically try to distract yourself with pleasant thoughts. That does not advance the stretch. The opposite works better: direct your attention straight to the hole and perceive the feeling as precisely as possible. You can go one step further: rate the pain on a scale of 1 to 10, assign it a color, a sound, a shape. Then feel into it again. Still on the same level?
- By connecting pain and breath, a technique of the yogis: focus on the pain and direct your breath toward it. With every exhale it becomes a little less.
- By overriding: instead of blocking out the pain, you overlay it with a stronger stimulus. This works best with a second person: when the pain grows, a surprising, intense stimulus somewhere entirely different on the body abruptly shifts attention.
- Through lust itself: besides all the pain, what turns you on about this stretch? Direct your attention exactly there, sink into the feeling, and expand it.

## Lust for stretching

Fisting consists primarily of a single sensation: the stretching of hole and bowel. If you take no pleasure in stretching exercises, it is unlikely that fisting will make you happy, because it consists primarily of **fisting stretching**. Not only stretching the hole, but also the bowel. In twelve men who had become incontinent after this procedure, Speakman and colleagues (1991) found a torn internal sphincter in eleven by ultrasound, extensively fragmented in ten. How high the risk actually was is shown by the figures in the overstretching section below. A meta-analysis of 148 randomized studies on fissure treatment (Nelson et al., 2017) rates manual stretching as inferior to the surgical alternative, including on the incontinence endpoint.

The difference from your training is the violence: there, hands under anesthesia tore open in minutes what no pain signal could stop anymore. Here, you stretch yourself, slowly, with full pain perception. That is exactly why switching off the pain is not an option, but we will get to that.

## How to practice

Avoid rough movements. They can cause tears in the mucosa, and tears are entry ports for pathogens that can trigger inflammation. So forget the porn routine and treat yourself gently. You win nothing against the natural limits of your body; you only pay for it. ## Aids for stretching

For stretching training you need three aids in this order: your fingers, a slim dildo or plug, and later an inflatable toy. Starting with your own fingers is the obvious choice, but it gets uncomfortable over time. And if you want to stretch up to a fist, fingers eventually take you no further. There are aids.

One rule applies to every toy you insert: it needs a wide base, wider than the shaft. Without this foot, the toy can disappear completely into the bowel. This is not scaremongering but emergency-medicine routine. A US survey estimates the number of emergency room visits for retained rectal foreign bodies between 2012 and 2021 at almost 39,000. More than half of the objects were toys (Loria et al., 2023). A systematic review found that 45 % of removals were done under general anesthesia and almost every third case had complications (Ploner et al., 2020). Honestly speaking: the base rule is consensus without its own study; only the problem it prevents is documented.

## Training on a psychological level

Half of stretching training happens in your head. For stretching to succeed, openness is required. Many of us were once taught that the anus is dirty and that we will be punished for playing with it. Anyone still guided by such prohibitions will find it hard to enjoy anal play. You overcome these inner prohibitions by making positive experiences and letting them gradually overwrite the old ones. That works by lovingly exploring your own anus, playing with it, and fostering pleasurable sensations. In a randomized study by Rao and colleagues (2007), targeted relaxation training resolved the cramping significantly more often than sham or standard therapy. An Italian study (Chiarioni et al., 2006) found 80 % clearly improved under biofeedback after six months, versus 22 % under laxatives. The international expert consensus (ANMS/ESNM, 2015) rates the method with the highest level of evidence, Level I, Grade A. The patients there, however, were mostly women with constipation, not men having sex. The serious transfer reads only: learning to consciously relax the pelvic floor is possible, and training changes the measurements.

## Turn off the pain

Why not simply switch off the stretching pain so the other can get in faster and get going? After all, people take painkillers to go to the office despite a headache.

Quite simply: pain is an alarm signal. If you switch it off, you are smashing the car’s warning lamp that lights up when the oil runs low. The engine still seizes up; you just no longer notice in time. On the human body, however, the hole cannot be replaced. Anything that dampens the pain signal, whether spray, pill, or drug, does not make stretching easier, only more dangerous. ## Fist pain

Two material properties decide on your toy: softness for the hole and firmness for control. Let us start with a bit of materials science. To keep stretching, you need to build a little pressure, and with a bendy toy in an awkward position you will soon give up in frustration.

Hard dildos allow more pressure and thus better control over your progress. In return, their use carries the risk of injury if inserted impatiently. You see, both the flexible and the hard ones have their pros and cons. Try out which ones you get along with better.

## Colours

The color of your toy is less about looks than about practicality. Think carefully about whether you want to own flesh-colored dildos, because they show every stain and every edge of discoloration. This is not about a beauty contest but about a stretching effect. Dark colors are the easier-care choice and still look new after a hundred training sessions.

## Vegetable dildos

Vegetables are ruled out as toys for three reasons: they can break, they can irritate, and they can disappear into the bowel. We therefore advise against vegetables. Cucumbers can break. Carrots have nice ridges but are too thin. The surface of zucchini is covered with fine hairs that can cause considerable irritation. And remember the emergency room statistics above: anything that can break or disappear completely does not belong in the bowel.

## Shapes

The most important shape rule is already in the aids section: only toys with a wide base. As for shape otherwise: models with a gentle, continuous thickening are best for stretching training, because you can increase the diameter step by step, just like when pumping.

## Care instructions

Caring for your toys consists of three steps: washing, drying, and storing them properly. Wash your toys by hand after every use with warm water and mild soap, and let them dry completely. Follow the manufacturer’s care instructions; some materials tolerate more, some less. ## Positions for insertion

A word about positions: as a beginner you tend to insert the things lying down, mostly on your side. The thought is understandable; relaxing is easiest there. You have better control over these beasts, however, when you sit down on the toy. Place it on the floor (some come with a foot or suction cup), squat, and lower yourself onto it. That way you regulate pressure and penetration depth with your legs. Lying down, the things slip away, and not everyone is flexible enough to apply the necessary pressure from behind. ## Training plan

This plan guides you through a complete stretching session in ten steps.

- Although not strictly necessary for stretching, douche your rectum to get practice in that as well. If needed, a [homemade anal douche](https://gay-blog.tomrockets.com/en/anal-douche-digestion-tips/) will do.
- Start the exercise with a hot shower or a relaxing bath.
- Prepare a warm, quiet spot where you do not have to worry about mess (a pad!).

- Initially slide one, two, three fingers past the sphincter.
- Then practice best in a squat by lowering yourself onto a toy with a base. That gives you the most control.
- Grease the toy thickly as well (it is never [too much](https://gay-blog.tomrockets.com/en/too-little-sex-too-much-sex/ "too much")).
- Sit down, pull your buttocks apart, press from the inside as if having a bowel movement, stretch until just before pain, wait, relax, wait more, relax even more, then stretch further. And wait again.
- Reapply lube in between.
- Practice at least twice a week, three times is better, and progress will come.

## Exercise duration

Keep reapplying lube! But the human hole is also subject to the laws of physics: at some point it reports fatigue. That is when you should stop at the latest. At the beginning this point comes sooner rather than later. Clearly, at first pain comes before pleasure, and it takes a while for the ratio to shift. ## Overstretching

The risk of overstretching is measurable, and the figures come from surgery. But it is equally wrong to downplay the risk. The data draws a clear line: the documented sphincter damage came from forced stretching under anesthesia, meaning within minutes and without a pain signal. In the Nielsen study, 65 % of those examined afterwards had defects in the muscle, and 12.5 % reported minor incontinence. The kicker: even 11 of 18 clinically continent patients showed damage on ultrasound. The muscle can suffer without you noticing. Slow, self-controlled training with dilators showed no incontinence in the fissure studies, though without ultrasound follow-up.

The honest summary: if you respect your body’s limits, severe consequences are unlikely, and as with any muscle training you even learn to control your sphincter better. That slow training leaves no trace at all is, however, unproven, because the studies without ultrasound cannot rule out small damage. If you overstretch, you are in any case gambling with the seal that keeps you dry for a lifetime.

## Training duration

The scene’s rule of thumb says three months of [preparation](https://gay-blog.tomrockets.com/en/anal-sex-preparation/ "preparation") up to the fist; no study on this exists. The answer can only be vague, because the starting conditions are too different. Every pelvic floor is different, every psyche is different, and previous experience plays a role. Someone who has enjoyed big cocks for years will probably find relaxing easier than someone just starting out. And progress depends heavily on how often you practice. As a rule of thumb from the scene: expect about three months of preparation. No study has measured this. The faster you want it, the longer it takes. ## The consequences of stretching

Since the hole is a muscle, it contracts again by itself. If you treated it carefully, all stretching sensations should disappear within hours, by the next day at the latest. During this time: do not postpone the toilet at the first sign of the urge. If pain, blood, or an unclean feeling persists longer, that is a reason for the doctor rather than the next exercise. And because it matters so much, one last time: warning signals are not meant to be numbed.
