A variety of **[fisting](https://gay-blog.tomrockets.com/en/fisting-stretching/ "fisting") positions** lets you adjust depth and intensity during a session. From the sling to doggy style: here are the most common positions with their real pros and cons. Honest, with actual research instead of scene folklore. That fisting came with the slings is how Knut Koch, actor and author of the memoir \*Barfuß als Prinz\*, remembers it. And back in 1977, Charles Silverstein and Edmund White described in \*The Joy of Gay Sex\* how differently men lie and kneel for it. What almost nobody has done since: check which of the common claims about positions and anatomy actually hold up. That is exactly what we are doing here, with everything research has to offer.

## Headfirst into happiness

Perhaps one of the best fisting positions of all: stand up and slowly lower yourself onto a vertical forearm. The „best“ in the sense of achieving something that seemed impossible before. It is absolutely legitimate to make yourself comfortable during sex. That wish is especially natural when you plan to be worked on for a while. But no comfort comes for free. Every position changes three things at once: the angle at which the hand enters, the control the bottom has, and the communication between you. We go through the most common positions and tell you what they can do and what they cost.

## What anatomy has to do with the position

Before we get to individual positions, a look under the hood. Because most of what scene guides say about anatomy is wrong or made up.

At rest, your anal canal sits at an angle of roughly 90 degrees to the rectum. This bend is held by a muscular sling, the puborectalis, which loops around the junction from behind. For anything to move in or out, this angle has to open up. Two things make that happen: the sling relaxes, and hip flexion increases.

A Japanese research team led by Sakakibara measured this in 2010, albeit during defecation, not sex: during straining, the angle was 100 degrees in a normal sitting position and 126 degrees in a squat. The authors conclude: the greater the hip flexion, the straighter the path and the less pressure is needed. A study from the United States on the toilet footstool (Modi et al., 2019) found a matching result. With raised knees, the bowel emptied significantly more completely and with less straining, across 52 participants and more than 1,100 documented bowel movements.

To be honest, the caveat belongs here: all of these studies look at defecation, not penetration. That the same mechanics help during fisting is a plausible transfer, not a measured fact. In practice it means: knees toward the chest is the lever, not gravity. And lying on your back is actually the worst position for the bowel’s exit. In a study by Rao and colleagues (2006), 60 percent of participants lying down failed to expel a balloon from the rectum, while only 16 percent failed when sitting. A second anatomical point matters more than any question of position: the junction from the rectum to the sigmoid colon. That is where the flexible passage ends, and that is where the serious [injuries](https://gay-blog.tomrockets.com/en/fisting-injuries/ "injuries") happen. In a case series by Barone and colleagues (1983) [covering](https://gay-blog.tomrockets.com/en/bdsm-covern/ "covering") 101 patients, exactly ten rectosigmoid perforations required surgery, above the peritoneal reflection usually with a colostomy. A case from Amsterdam (2012) shows how far up this can happen: the bowel wall tore 40 centimeters beyond the anus. Anything that goes past the wrist into depth play operates in this risk zone. ## In the sling or on the bed

The sling is the classic piece of fisting furniture. It vaguely resembles a hammock, but lets you lie relaxed on your back while your legs are held up in straps and out of the way. No serious large-scale event goes without one, and many saunas are equipped with them.

The setup also offers advantages for the top. He sits comfortably in front of you and can focus on his work with both hands. It gets anatomically interesting when you ask why it works. Transferred to fisting, this means: the sling turns the result around because your legs are raised and your knees are pulled toward your chest. Exactly this hip flexion is what opens the angle between anal canal and rectum. So it is not the lying that works, it is the leg position. You can use this on a bed as well: pull your knees up far instead of lying flat.

The position still has two drawbacks. The first is control: as the bottom, you are pretty much at the top’s mercy on your back. You cannot pull away or ease off if pain strikes unexpectedly. You can speak, call out, tense your muscles, but in a practice where millimeters sometimes matter, you control very little yourself. The second concerns depth: going too far here quickly reaches the junction to the sigmoid, exactly the zone where the serious injuries occur. In the sling, depth is a decision the top makes, not an [accident](https://gay-blog.tomrockets.com/en/anal-sex-accident/ "accident"). Trust and negotiation are not accessories here, they are the safety equipment.

## The doggy position

The alternative: doggy style. You go on all fours, supported on knees and hands or knees and elbows. That brings solid advantages. Unlike lying on your back, you can actively participate as the bottom. Push onto the entering hand or pull back from it. Move left and right, arch or straighten your back. That way you guide the top with your whole body.

Here, too, the biomechanics from the first section help. Lower your chest and shoulders and the hip flexion increases further. The angle improves, the path straightens. Whether penetration is actually easier and deeper in this posture has, as said, never been measured directly. But it is the position where the measured mechanics work best in your favor. Beginners in particular find it easier this way. On top of that, the upright spine is easier on your back, and you can loosen tense muscles with small movements.

The drawbacks: you have no eye contact and depend on words, unless you are so attuned that the top feels exactly where you want him. In instinctive moments, words are sometimes missing. A mirror at the head end helps. And sitting behind you on the floor for a long time is uncomfortable for the top. It gets easier if you kneel on the bed and he sits on a chair. Then, however, your lower legs dangle unsupported. ## The sideways position

Many recommend the sideways position especially for getting started, because the pelvic floor relaxes easily in it. That is plausible, but be honestly warned: this has not been measured either. It is simply one of the most comfortable positions there is, and comfort relaxes.

The sideways position works well for stretching exercises. To get a whole hand past the sphincter, you need to lift the upper leg. Going really deep will be difficult, because the top would have to twist his wrist considerably. Eye contact is impossible in this position, and control is limited as well. ## Position on the belly

A rather problematic position for many. As the bottom, you will struggle to keep your head comfortable for long. The fister has to work hard to thread his way around the tailbone.

Then there is the hip mechanics: lying on your belly, your hips are extended, and the exact opposite of hip flexion is the exact opposite of an open angle. Of all the positions here, the belly position works most strongly against the anatomy, going by the derived mechanics. Unmeasured as that is, too. Beginners who have no orientation yet will probably give up quickly. That is fine, too.

## When standing

The idea does not occur to you at first: as the bottom, simply stand there while getting fisted from below in various **fisting positions**. Relaxing the pelvic floor while standing seems exhausting. And indeed, this is more of a position for the experienced.

But it offers decisive advantages: as the bottom, you have full control over how deep the hand goes. You regulate everything with your legs. Opening up even gets easier when you let yourself sink onto the hand. Bend your upper body forward and you add the hip flexion that opens the angle. Sitting on the floor may not be quite as comfortable for the top, but he can prop his elbow on his knee. Eye contact is difficult but possible. And in front of a full-length mirror, you can both see more than in any other position.

## Upside down – Fisting positions

It sounds tempting: to reach the deepest depths, hang your partner upside down by the legs. Many guides claim that gravity straightens the bowel in this position. That is simply false. The only documented mechanism that straightens the path into the rectum is hip flexion, and that does not require a person hanging headfirst.

What gravity actually does to you upside down has been studied medically, on the eye and on circulation. Friberg and Weinreb (1985) measured in the Journal of the American Medical Association an intraocular pressure of 35.6 instead of 14.1 mmHg in healthy volunteers hanging headfirst. That is more than double and right in the range considered risky for glaucoma. The authors expressly advise anyone with glaucoma, elevated eye pressure, or retinal disease to avoid hanging upside down altogether, and they recommend that even healthy people avoid prolonged periods in the position. In an earlier study by the same group (Weinreb et al., 1984), pressure in glaucoma patients rose from 21.3 to 37.6 mmHg. Blood pressure and heart rate also rise measurably in the position (Souza, 1987). Anyone with blood pressure or heart problems should therefore be cautious as well.

Add the question of control and it gets truly uncomfortable: as the bottom, you have no control at all when hanging upside down, zero. You cannot move away, cannot dose, cannot get out quickly. It is also exhausting to hang head down, and you need load-bearing rigging plus at least one more person to help. Finally, the bottom must already be well relaxed or long trained before the top can enter this way, or he will be hanging too long. If you still find this position appealing: keep it short, secure it properly, and stop immediately if you feel pressure in your head, dizziness, or vision problems.

## Squatting

Much the same applies to squatting as to standing, except the top has to strain a little more. It is still interesting: the squat is the only position for which the angle effect was actually measured, with 126 degrees the most open value in the Sakakibara study. Measured during defecation, though, and in just six subjects, not during sex.

In practice, squatting is mainly useful as an alternation with standing, simply because not everyone has the leg muscles for hours of standing in a bent posture.

## Self-fisting

The question about the best position for self-fisting comes up regularly as well. The honest answer first: there is no research on this at all, only experience. The practical limit is your own reach. Most people fail not on courage but on the anatomy of their arms.

What works best are positions with bent hips: squatting, kneeling with the upper body bent far forward, or lying on your back with your legs pulled up. A mirror helps, because in none of these postures can you see what you are doing. What you completely lose in self-fisting is the second safety instance: nobody watches force, angle, and depth from the outside. With depth in particular, that is a real risk, because you can reach the dangerous zone at the sigmoid junction on your own. ## Fisting positions and combinations

To round off the position questions: you can of course switch **fisting positions**. Especially during longer sessions, the bottom will want to change position to ease back or joint pain or to revive legs that are slowly falling asleep in the sling.

The honest answer: no study in the world has ever compared fisting positions against each other. All we have is the measured mechanics of the pelvic floor and the experience of the scene. From that, five criteria emerge that actually matter when choosing:

**Hip flexion.** The closer the knees come to the chest, the more open the angle at the bowel’s exit. This is the only effect with any measurement data at all, and those come from defecation research, not from the bedroom.

**Control.** Whoever controls the depth carries the safety. Standing, squatting, and doggy style put the bottom in control. In the sling and upside down, the top controls everything.

**Communication.** Eye contact replaces a hundred words. The sling and standing in front of a mirror offer it, doggy style and sideways do not.
