# How to Stay Safe When Fisting on Drugs

If you move in the [fisting](https://gay-blog.tomrockets.com/en/fisting-stretching/ "fisting") scene, drugs are part of the landscape. Around 16 % of men who have sex with men report [chemsex](https://gay-blog.tomrockets.com/en/bdsm-chemsex/ "chemsex") in a European meta-analysis. That means sex under drugs taken specifically for that purpose. And men who engage in chemsex also practice things like fisting and [BDSM](https://gay-blog.tomrockets.com/en/bdsm-fetish-kink/ "BDSM") far more often. In a French cohort from 2025 with nearly 2,500 participants, the odds for these practices were 4.7 times higher. This guide takes that reality seriously. We explain three things. First, what the common substances do in your body. Second, which combinations can be lethal. Third, what you do in an emergency. No scaremongering and no glamorizing, just facts.

One thing upfront, because it is the most important sentence in this guide. Pain is your protection signal when fisting. It tells you when to stop, before tissue tears. Almost every drug on this list dampens exactly that signal. That is why fisting on drugs is riskier than other sex on drugs.

## What drugs do

Drugs extend sessions and change what you feel. In fisting, that is a dangerous combination. And it is not just theory. In 2004, the journal [Sexually Transmitted Infections](https://gay-blog.tomrockets.com/en/health-en/sexually-transmitted-infections/ "Sexually Transmitted Infections") published a case series from the Kobler Clinic in London. It describes three men with HIV. All three arrived at the emergency department with bowel perforations after fisting parties. All three had used ketamine. Depending on the case, [poppers](https://gay-blog.tomrockets.com/en/poppers-en/ "poppers"), ecstasy, or cannabis came on top. The most severe case was a 54-year-old. His partner had inserted his hand, arm, and a 15-centimeter dildo up into the sigmoid colon. The man felt a punch in his abdomen but no clear alarm signals. He only reached hospital 36 hours later, with two holes in his colon and peritonitis. The doctors drew a clear conclusion. Drugs prolong sexual activity and thereby increase the risk. The men sustained serious [injuries](https://gay-blog.tomrockets.com/en/fisting-injuries/ "injuries") without noticing the symptoms of the perforation.

That is why we now go substance by substance through the honest picture: effects, risks, and what each one means for fisting. Fisting itself is no niche, by the way. In a large British survey of gay men, 12.8 % said they had practiced fisting in the past year.

### Poppers

Poppers are volatile nitrites that you sniff. The effect hits within seconds and fades after a few minutes. You feel warmth, a racing heart, dizziness. On top of that comes a short window in which smooth muscle relaxes. That includes the sphincter, and that is why bottoms like poppers for the entry phase. The risks are in the fine print. Your blood pressure drops, in some people so hard that nothing works at all. Rare but documented are methemoglobinemias, in which the blood can no longer transport oxygen properly. Also documented is damage to the retina, so-called poppers maculopathy. Eye specialists published new case reports on it as recently as 2025. By far the most dangerous combination is poppers plus ED medication like Viagra, more on that below. ### MDMA

MDMA, on the street ecstasy, opens the heart, or so it feels. Closeness, trust, and touch feel more intense. Medically, MDMA is an amphetamine derivative, and you should know the two documented mechanisms of death. The first is overheating, medically hyperthermia. MDMA drives body temperature up. On a dance floor or during hours-long sessions, that can end in seizures and organ failure. The second mechanism is barely known and hits precisely the cautious ones: water intoxication. The medical term is hyponatremia. MDMA makes the body retain water. If you then drink liters out of fear of dehydration, you dilute the sodium in your blood until the brain swells. A British case series from a poisons information service counted 17 such cases with two deaths. In Sweden, a young woman died after a single tablet because she drank [too much](https://gay-blog.tomrockets.com/en/too-little-sex-too-much-sex/ "too much"). Do drink, but in moderation, not bottle after bottle.

Dosage is surrounded by myths. A fixed toxic threshold, the kind that circulates online, does not exist. Safer-use organizations like saferparty in Zurich go by body weight. The upper guideline is roughly 1.5 milligrams per kilo for men and 1.3 for women. And redosing is pointless. Safer-use organizations warn: the effect cannot be increased, only the side effects. One of the three cases in the London series was a 37-year-old at a fisting party. He had combined poppers, ecstasy, and ketamine. He ended up with a ten-centimeter tear in his rectum and 600 milliliters of blood in his abdominal cavity.

### Crystal Meth

Crystal meth, usually called Tina in the scene, is the substance experts name first when it comes to chemsex. The amphetamine keeps you awake for days. It switches off hunger, fatigue, and pain, and it cranks libido and stamina to the maximum. Exactly that mix makes it so treacherous for fisting. You notice neither that your body is done nor that your bowel has had enough long ago. Add the psychiatric risks: paranoia, hallucinations, full-blown psychoses. The [addiction](https://gay-blog.tomrockets.com/en/addiction/ "addiction") potential is high. Whether snorted, smoked, applied rectally, or injected: Tina is cardiotoxic. Heart attacks and arrhythmias are the most common fatal complications. And here is the point for fisting: Tina does not relax you. It makes you more awake and harder, not looser.

### Ketamine

Ketamine is an anesthetic that decouples you from your body. Experts call this dissociation. You perceive pain, but it no longer belongs to you. In fisting, that is the worst case, period. The 54-year-old from the London case series had snorted ketamine, and so had the first case in the series. At higher doses, space and time dissolve. In an [overdose](https://gay-blog.tomrockets.com/en/overdose-on-poppers/ "overdose"), you fall into the so-called K-hole. Inside it, you perceive nothing and cannot move. Ketamine also carries a long-term risk almost nobody knows. Used regularly, it destroys the bladder. These bladder damages have been their own clinical diagnosis since 2007, ketamine cystitis. In a British survey, 26.6 % of regular users reported urinary tract symptoms. Case series from Hong Kong show courses up to dialysis dependency. In 51 % of those affected, the symptoms improved after stopping use.

### Cocaine

Cocaine is a local anesthetic. It numbs the spot it touches. Exactly that property makes it popular in the scene and doubly dangerous for fisting. Applied anally, it numbs the hole while simultaneously constricting blood vessels and revving up the circulation. The American Journal of Forensic Medicine and Pathology documents the death of a 29-year-old. His partner had applied cocaine to his rectum, explicitly to relax the sphincter. The German AIDS service organization also warns that the acid in cocaine attacks the intestinal lining. And you should know one thing about the route. Through the bowel lining, the effect hits the blood almost as fast and as hard as an injection. Your ass is no overdose protection. The fact that cocaine is frequently cut, among other things with the anesthetic lidocaine, makes the effect even less predictable.

### GHB / GBL

GHB (gamma-hydroxybutyrate) and its precursor [GBL](https://gay-blog.tomrockets.com/en/emergencies-with-ghb-gbl/ "GBL"), known in scene slang as liquid ecstasy, have nothing to do with ecstasy. They are heavy sedatives with a treacherous property: the dose-response curve is extremely steep. The distance between „nicely high“ and „unconscious with breathing problems“ is a few milliliters. There is no antidote, toxicology states that explicitly. GBL acts even faster and stronger than GHB, because the body converts it rapidly. Combined with alcohol, the mix can lead to respiratory arrest. Taken daily, GHB causes physical dependence. The withdrawal can be life-threatening and belongs in a hospital. A small correction to what is often claimed online: GHB has not completely disappeared from medicine. As sodium oxybate, it is licensed in the EU for the sleep disorder narcolepsy. And because GHB is colorless and nearly tasteless, it also plays a role in date-rape drug incidents. Never let your drink out of sight at a party.

### Cathinones

Cathinones like mephedrone and 3-MMC are synthetic stimulants and part of the basic inventory of the chemsex scene. On the legal situation, because outdated info circulates: the gray area is history. Mephedrone has been a controlled narcotic in Germany since 2010. The entire substance group has been covered by the New Psychoactive Substances Act since 2016. 3-MMC has been controlled EU-wide since 2022. The real risk, though, is the composition. Nobody knows what is actually in that powder or how strong it is. That is exactly what drug checking exists for, more below.

### Cannabis

Weed relaxes, but it also makes you tired, sluggish, and in some people paranoid. Risk-free, as often claimed in the past, it is not. Psychiatric side effects are real. For fisting it is rather unsuitable. You want to be neither sedated nor anxious when someone has a fist inside you.

### LSD

LSD is a classic psychedelic with eight to twelve hours of duration. For a [fisting session](https://gay-blog.tomrockets.com/en/eat-before-a-fisting-session/ "fisting session"), the time logic alone makes it unsuitable. One scene claim needs correcting here. The idea that LSD relaxes the involuntary muscles and „holds on to“ inserted objects is hearsay without any evidence. In animal pharmacology, LSD tends to do the opposite to smooth muscle. The sphincter relaxation people mean is a poppers effect. What LSD really does: dissolve perception and ego [boundaries](https://gay-blog.tomrockets.com/en/bdsm-limit-boundaries/ "boundaries"). In an environment where you are physically vulnerable, that is a recipe for bad trips.

## These combinations are especially dangerous

On their own, the substances are risky; mixed, they become unpredictable. Experts call this drug interactions, and they are the underestimated part of polydrug use. Four combinations are so well documented that we name them individually.

**Poppers plus ED medication.** Viagra, Cialis, and the like lower blood pressure, and so do poppers. Both act through the same mechanism. Together, blood pressure can drop so far that heart and brain are no longer supplied. The European Medicines Agency lists combining sildenafil with nitrites explicitly as a contraindication. That is not gray theory but the official package leaflet.

**MDMA or GHB plus HIV medication.** HIV is well controlled today, but the therapy has a catch. If you take medication with the boosters ritonavir or cobicistat, your body breaks down MDMA far more slowly. The Lancet documents the death of a 32-year-old who took an ordinary ecstasy dose while on ritonavir. His MDMA blood level was several times higher than expected. A second case report describes a patient who took GHB on top of MDMA while on these medications and had to be ventilated. If you take HIV medication, this is the most important line in this guide. **GHB plus alcohol.** Two respiratory depressants amplify each other. This combination regularly ends in ambulances. Unconsciousness, [vomiting](https://gay-blog.tomrockets.com/en/blowjob-vomiting/ "vomiting") while lying down, respiratory arrest.

**Ketamine plus GHB.** Frequently combined in the scene, examined in the lab. In animal models, ketamine slows the breakdown of GHB and markedly increases lethality. Human studies are still missing. In a scene survey, 30 % of GHB users said they combine it with ketamine. None of that changes the fact that both substances can knock you unconscious.

## What to do in an emergency

When something goes wrong at a session, minutes count. You need to know these warning signs: feverish overheating with hot, dry skin, seizures, chest pain, severe confusion or panic. Plus a person who cannot be woken, slow or pausing breathing, bluish lips.

Then the rule is: call emergency services, 112 in the EU, 911 in the US, 999 in the UK. Place an unconscious but breathing person in the recovery position. Cool them if they are overheating, and stay with them. Tell the paramedics honestly what was taken. That information can decide between life and death, and in many countries, including Germany, the emergency doctor is bound by medical confidentiality. The German AIDS service organization recommends exactly that explicitly: give the address, name the drugs taken. Failing to help, by contrast, can be a criminal offense. The keyword is failure to render aid. And one correction to a persistent piece of scene advice. Benzodiazepines like lorazepam are indeed the hospital treatment of choice for severe agitation after stimulants. But that is a decision for the emergency physician, not for a buddy’s medicine cabinet. Your job is the emergency call, not self-medication.

## Safer use: if you use anyway

Safer use is the principle of harm reduction: you use, but as controlled as possible. A guide like this one will not stop you from using, and that is not its job. Its job is that you do not die or wake up with a stoma. So here are the most important rules.

**Get your substance tested.** In Berlin, you can have drugs analyzed legally and free of charge since 2023, including at the gay counseling center. The address is drugchecking.berlin. In Zurich, saferparty has done it for years, in Vienna checkit!. You learn what is actually in your stuff and how strong it is.

**Rectal is not the harmless route.** The German AIDS service organization stresses it in its „Up your bum“ portal: through the bowel, a substance acts almost as fast and as hard as injected. If you use rectally to be „safer“, you are wrong. The overdose comes just the same, only without a needle.

**Dose low and do not redose blindly.** Wait for the full effect before taking more. GHB and MDMA in particular punish impatience.

**Drink in moderation, not in volume.** With MDMA, both are true. Dehydrating is dangerous, and so is too much water.

**Negotiate boundaries sober.** Safe word, clear agreements, ideally someone present who stays sober and makes the emergency call if needed.
