What separates someone who simply has [anal sex](https://gay-blog.tomrockets.com/en/fucking-anal/ "anal sex") from someone who has mastered it? The good news first: the difference is measurable, and it has surprisingly little to do with anatomy. It went from 41 percent among those with fewer than ten experiences to 92 percent among those with more than five hundred (Gaither et al., 2023). At the same time, strong insertion pain dropped from 39 percent to 13 percent, and the sensation of rectal urgency fell from 21 percent to 6 percent.

The caveat comes with it: this survey is cross-sectional. It compares experienced with inexperienced people instead of following the same people over time. Still, it is the best available number on the learning curve, and it matches the scene’s practical experience. ### Advanced Anal sex: Not a sprint, but a marathon

A controlled study on [anal stretching](https://gay-blog.tomrockets.com/en/anal-stretching-for-beginners-advanced/ "anal stretching") training as a sexual practice does not exist. Our PubMed search for „anal stretching training sexual“ returned two off-topic hits, and the terms „anal dilation“ and „anal dilator“ in a sexual context returned exactly zero. What competitors sell as a training plan is experiential knowledge without a research basis, and we say so openly.

What is documented is what decides between pain and pleasure. In a Belgian study of 1,752 gay men, 59 percent of receptive partners reported some degree of anal pain (Vansintejan et al., 2013). The strongest predictors were insufficient lubricant and missing oral or digital stimulation before penetration. A Polish survey of 1,443 non-heterosexual men added the psychological side: performance anxiety markedly increased pain intensity (Grabski & Kasparek, 2020). ## Analsex for “Pros” – Giant Dildos & Double Penetration

Pornography is not a training plan: a content analysis of 304 scenes from best-selling porn videos found physical aggression in 88.2 percent (Bridges et al., 2010). The performers‘ preparation stays invisible on screen: what looks like spontaneous extreme practice there is choreography. What happens when things go wrong, by contrast, is well documented. A systematic review of rectal foreign bodies in older men aggregated 582 patients (Yang et al., 2022). The most common reason was insertion for erotic motives. A ten-year single-center series counts 22 cases, 68.2 percent of them sexually motivated (Kokemohr et al., 2017). 18 were removed without anesthesia, and one case ended with a sigmoid perforation. ## How “anal sex professionals” massage the prostate to climax: Stimulating the G-spot!

The honest answer first: the prostate orgasm is not measured in the biomedical literature. Our PubMed search for „prostate orgasm“ returned exactly zero hits. That the prostate is an erogenous zone remains physiologically plausible and is supported by the learning-curve data, but no study exists that causally links prostate stimulation to orgasm.

In practice, that means the technique rests on experiential knowledge, not on a textbook. Scene knowledge holds that pressure on the anterior bowel wall reaches the prostate better than sheer depth; no measurement exists. ## Advanced Anal sex: Why is the doggy style so popular?

No academic study on position preferences in gay anal sex exists. Our search returned 77 hits for the topic area; among the 20 most recent, not one was a primary study on position preference. What exists are commercial surveys without peer review: a US survey of 1,490 adults (NapLab) put doggy in first place with 29.4 percent. The only survey with a gay subgroup turns the picture on its head: there, Lotus led with 22 percent and doggy sat at 16 percent (Bespoke Surgical, 2021).

Why doggy still works is biomechanics, not a study finding. The receptive partner controls depth through his back. The angle meets the anterior bowel wall, and the position lets the pelvic floor relax. That is a plausible rationale, not a measured fact, and we label it exactly as such. ## Why do experienced anal sex fans cum in the passive partner?

The psychological answer comes from 18 in-depth interviews from the pre-PrEP era with men who deliberately had sex without condoms. For them, [semen](https://gay-blog.tomrockets.com/en/produce-semen/ "semen") exchange was a central variable of the experience: closeness, trust, and conscious transgression (Holmes & Warner, 2005). What the health system reads as irrational risk, the participants experience as the peak of intimacy.

The risk side has hard numbers. Unprotected receptive anal intercourse with an untreated HIV-positive partner carries around 138 transmissions per 10,000 contacts (Patel et al., 2014). That is the highest single sexual risk of all. The combination of condoms and effective treatment of the positive partner lowered the estimated risk by 99.2 percent. Pre-exposure prophylaxis (PrEP) reduced new HIV infections by 44 percent in the iPrEx trial and by 86 percent in the British PROUD study. ### Anal-professionals: hygiene also for anal toys

Material beats detergent: in a small experimental study, HPV was still detectable on porous TPE in 40 percent of cases even 24 hours after [cleaning](https://gay-blog.tomrockets.com/en/sextoy-cleaning/ "cleaning"), on silicone in zero cases (Anderson et al., 2014). The study examined vaginally used [vibrators](https://gay-blog.tomrockets.com/en/vibrators/ "vibrators") of twelve women; transferring it to anal use is plausible but not directly proven. A second line comes from microbiome research: in a female sample, frequent toy use was associated with altered vaginal flora (Marrazzo et al., 2011). That too is a directional pointer, not an anal measurement.

Then there is the material question. An analysis of four commercial toys found hormonally active phthalates above warning levels in all four (Sipe et al., 2023). Communication is measurably linked to better sex: a meta-analysis of 93 studies with 38,499 people found an association between sexual communication and sexual satisfaction of r = 0.43 (Mallory, 2022). Decisively: the quality of communication clearly beat its frequency.
